Q: What was the biggest challenge you had to overcome this past year because of COVID-19? 

 

The biggest challenge we have faced is getting back up to speed. COVID-19 slowed down the volume of patients and we had to devise a new way to make communication safe for ourselves and our patients. Safety was our #1 concern and we stayed diligent with the always fluid CDC guidelines. Some other challenges we faced were creating a system for telemedicine, as well as setting people up for remote workstations. 


Q: Were there any partners or businesses that helped you along the way?

Microwize is our #1. They are always helpful, especially with applying their knowledge to research telemed companies for us. In addition, they made setting up remote workstations, personal computers, and even cell phone apps possible. SolutionReach was able to help us with patient communication. Vonage, which is our phone system, had a mobile app allowing our staff to conduct all business remotely. 


Q: How has COVID-19 changed the way you do business? 

Highland Ophthalmology tried to maintain a business-as-usual approach within the guidelines from the state and CDC. We adapted to our apps and working remotely, which has been a great benefit for inclement weather as well. Microwize made sure we could adapt quickly. Our billing office operated at 100%, with 75% of the team working remotely. Due to telemedicine and the apps, Dr. Huynh even started using the EMR charges function in Medisoft, which has been a great tool. 


Q: What measures does your practice take for reducing your risk of a cyberattack?

Microwize plays a big part in monitoring our server usage. We work with them making sure our remote users are monitored, while also being sure to deactivate old domain users. IP addresses are monitored, and passwords will get changed if we feel anything nefarious is occurring. All our computers are shut down when not being used as a deterrent to any hackers. We created a shared spreadsheet between our office and Microwize listing our hardware, cloud users added, and which users have remote setups. 


Q: How does your practice stay connected with patients? 

We have found that consistently being there for our patients allows them seemingly unlimited access to reach us. We use SolutionReach to remind our patients of upcoming appointments, and we are currently re-doing our website to allow for more patient accessibility. Our patients also know they can receive most care through the telehealth services we offer. We have created an office culture and atmosphere surrounded and created by patient communication to give our patients the best quality healthcare we can deliver. 


Q: In what ways are you preparing for the future of healthcare? 

We have prided ourselves in innovation and cutting-edge surgical procedures, and now we also pride ourselves in the latest ophthalmic technologies including new instruments. We adapted to Covid by adopting a new virtual visual field technology, which allowed for decreased contamination during the time spent in the office–all with virtual reality technology. We have recently updated and purchased new ophthalmic instruments to increase accuracy and create a better office flow as well.


We are currently updating our website to give our patients the freedom to book their own appointments. We are looking into creating patient portals as well as increasing the use of EMR charges. Dr. Huynh does an excellent job utilizing the Medisoft application. All of our doctors utilize the app to keep an updated, real-time schedule of their patients for the current and upcoming days.

Impacts of Telemedicine & Telehealth on Healthcare

Over 40 years old, telemedicine is a technological approach through which a healthcare provider can divulge health information from one location to their patients in another location. What is Cloud Computing?

Cloud computing, often referred to simply as ‘the cloud’ is the delivery of on-demand computing resources over the internet using a network of remote servers to store, manage and process data. It allows access to a shared pool of computing sources and one of the great benefits is that updates can happen quickly and seamlessly. This allows companies to avoid up front infrastructure costs and get their applications up and running much quicker, and they will require much less maintenance going forward.

Cloud computing is divided into three broad service categories:

  • Infrastructure as a service (IaaS) – this is where providers supply a virtual server, allocated storage, and application program interfaces (APIs) and can configure the system as needed providing for various workload needs.
  • Platform as a service (PaaS) – providers host development tools on their infrastructures and users access those tools over the Internet using Web portals or gateway software.
  • Software as service (SaaS) – a model that delivers software applications over the Internet and these are often called Web services. Microsoft Office 365 is a SaaS offering software and email services. Users can typically access these applications from any location using a computer or mobile device that has Internet access.

Cloud computing has become a highly demanded service due to the advantages of high computing power, cheap cost of services, high performance, scalability, accessibility, and availability.

EHR vs. Paper: Are physicians more efficient?

Visiting the doctors’ office usually is not an outing that one looks forward to all year. A simple patient appointment for a referral or common illness diagnosis can take hours primarily due to the wait time. Whether you are sitting in the waiting room or the examination room, the time to wait until you see your physician is significant. Speaking from the patient side of the equation, promoting the use of electronic health records versus paper allows this waiting time in between appointments to reduce. This is due to the simple fact that there is less work to be done after each patient. The argument about whether or not to go electronic vs paper has become more relevant to physicians as our healthcare system is undergoing major changes. Part of this change is the push for electronic health records.

Although some physicians prefer the manual method of paper and pencil, in a time where everything is digital, paper is not efficient anymore.

Paper is more costly over time

To process paper records, this requires additional employees to handle, support, and organize paper files and other documents. Despite the fact that electronic health records have greater costs initially, they reduce the cost of records significantly overtime.

Saves time

Physicians do not have to spend hours filling out endless amounts of paperwork. Employees do not have to waste time with the processing and handling of paperwork and finding places to store it. Therefore, the practice is better able to dedicate more time to the care of their patients. No more digging around file cabinets or taking time to drive to the warehouse to hunt for files. In emergency situations, every moment counts and now patients can get their needs fulfilled sooner.

Environmentally friendly

The storage of medical records typically exists in warehouses taking up space and deteriorating over time. This creates large amounts of unnecessary waste and is unhealthy for our environment.

Easily accessible

Both patients and physicians can access electronic health records from anywhere. Most electronic health records can be accessed from a patient’s home. This provides the patient a sense of convenience. Instead of calling or visiting the doctor’s office every time they have a question about their health record or a loved one’s, all they have to do is bring it up on their home computer.

Better organization

Paper records tend to be incomplete after being transferred to various medical facilities causing the likelihood of redundancies in tests in treatments to be greater. Having health records on line allow for a comprehensive medical history for all patients. Transferring documents electronically is also more convenient, efficient and faster.

Although these points clearly direct us to the conclusion that electronic health records would in fact be more efficient for a physician, some physicians are hesitant to make the switch. Reasons include the adjustment period many practices would have to face resulting in financial loss and downtime they could experience. However, in time electronic health records will prove to be time and money saving.

Physicians Voted Trump…..why?

This physician voted for Donald Trump. Here’s why.

A physician who voted for Donald Trump gave strong reasons why she did. She hesitated to put her name because of the outcry the election has caused but only indicated that she is a female physician, Let’s call her Dr. Jane. She said she felt a great need to express why she voted the way she did.

Physicians Voted for Trump

She practices independently with one other physician. In her words she said they are hanging on by a thread, but still hanging on. With a staff capacity of six, five full-time and one half-time employee who have been with them for years. The physician said, they have so far maintained health insurance for their employees but with decreased reimbursements as well as rising healthcare costs she has become pessimistic  not only on how well they’ll go on with not providing health insurance but also keeping their business as an independent practice. 

The physician was concerned that in January 2017 her health insurance is going to add to about 200 percent and also her deductible.  She’s not certain where the extra money would come from. Their practice attend to a relatively large number of patients every single day with great help of electronic health records, but with uncontrollable rising expenses such as rent, health insurance and utilities she not hopeful on how long they’ll remain independent.

The physician recounted her days in med school in the 1990s at the time when Hillary was charged with the duty of modifying the health system. She recollected that there was a fiasco that came with Hillary care which made a number of doctors close up shops and even her family doctor who retired early had warned her not to study medicine.

On a final note the Physician said she would have preferred if Elizabeth Warren or Bernie were elected and added that as a woman, she greatly respects Hillary and her career accomplishment but did not like her as president because of the catastrophic damage in the 1990s which resulted to dramatic increases in healthcare cost. She was emphatic, that she doesn’t want the same old thing happening again.

Population Health is an approach to health that aims to improve the health of an entire human population while reducing health inequities or disparities among different population groups and changing form individual based care to a model in which Population Health Management (PHM) plays a major role. The concept of population health came about in 2003 when David Kindig and Greg Stoddart defined it as “the health outcome of a group of individuals, including the distribution of such outcomes within the group.” There is some complaint that this first definition focuses only on the measurement of health outcomes without explaining the role that health care providers take to impact the outcomes.

MIPS and APMS – MACRA Payment Model FAQ

The Medicare Access and CHIP Reauthorization Act (MACRA) has been a newsworthy subject since it was signed into law on April 16, 2015. As well as the legislation repealing the Sustainable Growth Rate (SGR) and streamlining quality reporting programs into one system, it also establishes a new framework that will reward physicians for value over volume. Payment reform will quickly change Medicare and impact providers.

MIPS and APMS – MACRA Payment Model FAQ

Merit-Based Incentive Payment System, or MIPS, combines these older programs under one umbrella

  • The Physician Quality Reporting System (PQRS)
  • The Value-Based Payment Modifier (VBPM)
  • The Medicare EHR Incentive Program (Meaningful Use)
  • Comprehensive Primary Care Plus (CPC+)

Under MACRA, MIPS payments to providers with scores above the threshold in years 2019 – 2024 will receive proportionally bigger incentive payments up to three times the annual cap with a second threshold for exceptional performance set at the top 25% allowing the highest performing physicians to receive additional payments. But not all will see higher payments, some will see lower payments. While MIPS is largely rooted in ‘Fee for Service’ but modified to tie in ‘Pay for Performance’, whereas the Alternative Payment Model is just a ‘Pay for Performance’ Model.

Under Alternative Payment Models (APMs), from 2019 – 2024, some participating health care providers will be paid a lump-sum incentive payment, there will be increased transparency of physician-focused payment models, and starting in 2026, some participating health care providers will see higher annual payments. Payers, policy makers, and health care organizations are working to define APMs, but the underlying theme will have to do with efficiency and higher quality care provided at a lower cost.

Starting in 2019, providers can choose to base their payment updates upon successful participation in either MIPS or APMs and will be able to decide annually which program is best for them. As we move toward these new Pay for Performance Models, you can prepare by participating in PQRS by reporting quality measures through a qualified registry, and by using a certified EHR.

Interoperability Isn’t Just a Matter of Sharing Data

The Office of the National Coordinator (ONC) has been called on to rethink interoperability. Rather than just a gauge of data sharing, The American Medical Association (AMA) and other medical groups would like to see interoperability be a measure of the usefulness, timeliness, correctness and completeness of that data.

AMA President, Steven Stack, recently stated, “Vendors have been incentivized to meet the flawed benchmarks under the Meaningful Use program. We need to replace those benchmarks with ones that focus on better coordinated care. MACRA offers that opportunity and we need to take advantage of it.” He feels that the current measures should be done away with and new measures set under MACRA. The American Hospital Association (AHA) agrees stating that they would like to see expansion of the scope of measurement which would include standards to make sure technology and necessary infrastructure are in place. On July 1st, and as part of MACRA, The Department of Health and Human Services (HHS) is to provide reporting regarding metrics for EHR adoption.

Back in February, the ONC released the 2016 Interoperability Standards. The standards and implementation specifications that were listed focused explicitly on clinical health IT systems’ interoperability. Fast Healthcare Interoperability Resources (FHIR) also focuses on the technology behind the data exchange which is at the core of interoperability.  But as more doctors and organizations weigh in, it is clear they would like to see a practice that produces metrics that enhance the wellness of patients, and not focus on just data sharing.
